El Paso, Texas -- A polyurethane foam company has been fined following a fatal accident at its work site last year.
The US Occupational Safety and Health Administration (OSHA) started its investigation into Spray Polyurethane Foam after an employee died when he fell almost 30 ft (9 m) at the company's worksite at 9600 Plaza Circle, according to 25 Jan OSHA statement.
The investigation found that the roof of the facility did not have the required strength and structural integrity for repair work to be performed.
The company was fined $8700 for its safety violations. LMH"
