New Haven, Connecticut -- A urethane coating and laminate manufacturer in New Haven, Connecticut has been fined $74 000 by the US Occupational Safety and Health Administration (OHSA), according to reports in the local press.
Uretek Archer was penalised for safety violations following a fire at its site. The fire started as a machine was being cleaned with flammable solvents, The Hartford Courant reported. The machine did not have a system for locking out power while being cleaned, an OSHA requirement.
The OSHA report cited four serious safety violations - an inoperable fire alarm, lack of protective equipment for workers, inadequate fire extinguisher training, and the lack of fire precaution procedures related to cleaning machines, the newspaper said.
Four workers were hospitalised after the fire.
